Changing the health and social services centre

As a client of public health care, you can choose your own health and social services centre anywhere in Finland. Changing the health and social services centre requires submitting a written notification. Responsibility for your treatment will be transferred to the new health and social services centre no later than three weeks after the notification has been received.

The change of health and social services centre applies to health care services. Social services are directed according to the municipality of residence as derived from the population register data. Health care services include child and maternity clinics, dental care, mental health and substance abuse services, rehabilitation, oral health care, and short-term inpatient care at the health care centre. The services of the health and social services centre are provided in cooperation between the social welfare and health care services.

You do not get to choose your provider of school and student health care, long-term institutional care or home care.

You can change the health and social services centre up to once a year.

Return the completed form to both your current (old) and your chosen new health and social services centre.

If you have chosen a health and social services centre in another municipality of the Wellbeing Services County of Pirkanmaa and intend to use digital services at the chosen health and social services centre, you must update the information about the change of health and social services centre also in the wellbeing services county’s digital services under Personal information: OmaPirha service (in Finnish)
